How to Apply

There are several ways you can apply for Medicare.

  1. Enroll online. The simple step-by-step process guides you through the enrollment form.

  2. Call 866-973-4588 (toll free) or TTY 711 to speak with a licensed sales specialist. You can apply by phone or schedule an appointment to apply in person.

  3. At a Straight-Talk Seminar in your neighborhood.

  4. Download the enrollment form, and mail the completed form to Kaiser Permanente - Medicare Unit, P.O. Box 232400, San Diego, CA 92193-2400. Enrollment forms are also available in Chinese and Spanish. Be sure to keep a copy of the form for your records.

You will need the following documentation to enroll:

-Information on your Medicare card or letter from Social Security or Railroad Retirement Board
-Your current health plan information, including group or Medicaid (Medi-Cal in California) ID numbers (if you have Medicaid or your state helps pay for your Medicare premiums)
-Any other verification that shows you're entitled to Medicare Part A and enrolled in Medicare Part B
