AAP Food Samaritans is a food voucher program for terminally ill individuals. Vouchers are redeemable at Statler Brothers supermarkets for food, beverages, household needs (e.g., paper products), and toiletries. They may not be used to purchase alcohol, tobacco products, or pet supplies. Assistance is provided for 6 months to 36 months, dependent on client need.
-Must be diagnosed with a terminal illness, such as HIV, AIDS, cancer, liver disease, heart disease, or Multiple Sclerosis (MS).
-Must live within a 25 mile radius of Palm Springs.
-Must have gross annual household income at or below 150% of the federal poverty level.
-Must have maximum of $2,250 in countable resources (e.g., bank account, IRA) per household. Households including at least one member who is at least 65 years old or disabled may have up to $3,250 in countable resources. Homes and motor vehicles are generally not included as countable resources.
-Must submit to credit check.
-Must be drug free and sign an authorization for random drug testing.

How to Apply
Drop off a completed application along with all required documentation to begin the intake process. Required documentation includes:
-Current government issues photo ID (driver's license, state ID, or passport)
-Current lease or property tax bill
-Copies of utility bills
-Written diagnosis of a terminal illness from a health care provider
-All financial records for the past 12 months (e.g., pay stubs, income tax returns, bank statements, Social Security award letters, cable bills, etc.) for all household members. You must complete an IRS Form 4505 to request a copy of your income tax return.
-Section 8 letter, if applicable
-Rent receipts for the last 3 months, if you pay rent in cash
Program staff will review your application packet and contact you to schedule an in person interview. Due to budgetary constraints, a wait list for services may apply.
