How to Apply

Call 855-436-1234 to schedule an appointment. Walk ins are accepted for primary care services when space permits, but appointments are preferred. Specialty visits are by appointment only. You must bring the following for all appointments:

-Insurance card or Medi-Cal Card (if applicable).
-Immunization record
-All medication bottles of current medications,including any dietary supplements and/or vitamins.
-ID for proof of residence (if uninsured)

Please complete the patient registration packet prior to your appointment and bring it with you, or arrive early to complete the forms before your appointment. Forms are available in English, Spanish, and Arabic.
