When choosing a Cigna-Healthspring prescription drug plan (PDP), be sure to consider current medications, annual plan premiums, deductibles and coinsurance/copays. View the online drug list to determine if prescriptions are covered.
- For people eligible for Medicare, which are people age 65 and older, certain disabled people under age 65, people of any age with End-Stage Renal Disease (ESRD) (permanent kidney failure requiring dialysis or a kidney transplant).

How to Apply
To enroll in Cigna's Medicare Part D Plan call 855-391-2556 (TTY: 711), daily from 8am - 8pm. When you call please have your Social Security number and date of birth.
Enrolling by mail or fax is also an option. Download and complete the forms (English | Spanish) and send them to: Cigna-HealthSpring RX (PDP), P.O. Box 269005, Weston, FL 33326-9927, or fax them to 800-735-1469.
Medicare beneficiaries may also enroll in any of the Cigna-HealthSpring Rx Prescription Drug Plans through the CMS Medicare Online Enrollment Center.
